A Five Day Evaluation is for five weekdays. Typically a patient will be admitted on Sunday afternoon or early Monday morning. This enables us to complete the evaluation by the following Friday afternoon. If admission occurs on any other day, then the evaluation will require the patient stay seven days (to enable the patient to be present here for five weekdays.)
Components of the evaluation are:
* Medical History and Physical Examination
* Addiction Medicine Evaluation
* Psychiatric Evaluation
* Psychological Testing which includes:
* Clinical Interview
* Wechsler Abbreviated Scale of Intelligence
* Minnesota Multiphasic Personality Test
* Millon Clinical Multiaxial Inventory
* Rorschach Comprehensive System
* Baseline blood work, urine drug screen(s), and hair drug testing (if indicated)
* Psychosocial Assessment
* Therapist Evaluation
* Contacting Collateral Sources that know the patient (with HIPPA releases) for additional information
* Peer Evaluation in our Licensed Professionals Group
Contact with the Referral Source is made shortly after admission, again as needed during the week, and on the last day of the evaluation to discuss findings and recommendations before we meet with the patient to present a copy the letter we will be sending the referral source. A copy of this letter and salient supportive documents from the chart will be faxed to the referral source that same day and hard copies will be mailed. The patient is instructed to contact the referral contact person either immediately following presentation of our findings and recommendations or no less than one week day thereafter.
At the end of the evaluation, it is made completely clear to the patient that completion of the evaluation concludes his/her admission. The patient is told that he/she should contact his/her referral contact person to discuss our recommendations. If treatment is recommended, then he/she is told to discuss with the referral contact which centers are approved for treatment. The patient is told that admission to Palmetto for treatment is permitted, but that he/she is not being coerced in any way to receive treatment here. The patient may choose any approved center to receive treatment. If the patient wants treatment at Palmetto, then he/she is required to sign a consent form stating that he/she sincerely wanted treatment here and that there was absolutely no coercion or pressure to receive treatment here. If the patient does elect to continue in treatment here, the cost of the evaluation will be subtracted from the cost of the first month of treatment.
